Join Our Team as a Wellness Nurse (RN/LPN)

We are looking for a dedicated and empathetic Wellness Nurse (RN/LPN) to become a part of our Healthcare & Medical Services team within our assisted living and memory care community. This position is essential in ensuring the health and well-being of our residents.

Key Responsibilities

Conduct assessments of residents' health and wellness.
Administer medications and provide necessary treatments.
Develop and coordinate personalized care plans.
Work collaboratively with physicians, families, and care staff to ensure comprehensive care.
Promote resident well-being through health education and monitoring of any changes in condition.
Advocate for safety and comfort within the community.

Qualifications and Requirements

Current RN or LPN license.
Compassionate approach to patient care.
Strong communication and teamwork skills.
Ability to adapt to the needs of residents and their families.
