Hospice RN Case Manager

Rockstar is recruiting for a family-owned hospice agency with a 5-star rating from Medicare and national recognition as a “Superior Performer” by Strategic Healthcare Programs (SHP). This organization is known for providing compassionate, first-class care and is seeking a dedicated Hospice RN Case Manager to join its award-winning team. What You’ll Do Complete initial and ongoing assessments to determine hospice needs Create, update, and implement individualized care plans Deliver skilled nursing care, medications, and symptom management Support and educate patients and caregivers on comfort-focused care Serve as case manager for an assigned caseload Coordinate with physicians, families, and the interdisciplinary hospice team Participate in on-call rotation and team meetings Supervise and support hospice aides What We’re Looking For Graduate of an accredited nursing program; current RN license in Missouri (or eligible) 1-2 years of recent acute care experience (home health/hospice preferred) CPR certification Valid driver’s license and insured, reliable vehicle Excellent communication, assessment, and problem-solving skills Compassionate approach to end-of-life care Benefits Competitive pay Health, dental, and vision insurance Life insurance Paid time off Supportive, family-focused culture Schedule Monday-Friday, full-time Rotating on-call responsibilities Join a nationally recognized, family-owned hospice with a reputation for excellence.
