Position: Physical Therapist

Salary Range: $41-$43.50 per hour

Work Hours: 35 hours per week, Monday-Friday with weekend rotation and split/float schedule at Skyline Nursing and Rehab SNF

Job Summary:

We are seeking a highly skilled and dedicated Physical Therapist to join our team at Skyline Nursing and Rehab SNF. As a Physical Therapist, you will be responsible for providing exceptional physical therapy services to patients in our facility. Your main goal will be to help patients recover from injuries and illnesses, improve their mobility and relieve pain, and prevent future injuries. You will work closely with patients, their families, and a multidisciplinary team to develop personalized treatment plans and ensure the best possible outcomes.

Key Responsibilities:

- Perform initial evaluations and assessments to determine patients' physical therapy needs

- Develop individualized treatment plans based on patients' conditions, medical history, and goals

- Utilize a variety of techniques and modalities to help patients improve their mobility, strength, balance, and coordination

- Monitor and document patients' progress, adjust treatment plans as needed, and communicate updates to the healthcare team

- Educate patients and their families on exercises and activities to continue at home to maintain progress and prevent future injuries

- Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists, to ensure coordinated and comprehensive care for patients

- Adhere to all legal and ethical standards and maintain patient confidentiality at all times

- Stay current with advancements in physical therapy techniques and technologies through continuing education and professional development opportunities

Qualifications:

- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program

- Current state license to practice as a Physical Therapist

- Minimum of 1 year of experience in a clinical setting, preferably in a skilled nursing facility

- Strong knowledge of physical therapy principles, techniques, and modalities

- Ability to work independently and as part of a team

- Excellent communication and interpersonal skills

- Compassionate and patient-centered approach to care

- Flexibility to work weekends and split/float schedule as needed
