Physical Therapist Assistant

Job Summary:

We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required. Responsibilities:

• Provide skilled nursing care including patient assessments and medication administration.
• Develop and implement treatment plans in collaboration with healthcare providers.
• Manage case and hospice care for patients with advancing age, frailty, and serious illness.
• Collaborate effectively with the interdisciplinary care team to ensure optimal patient outcomes.
• Maintain accurate and timely documentation of patient care and treatment progress.
• Communicate clearly with patients, families, and healthcare professionals regarding care plans and patient needs.

Required Skills & Certifications:

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of two years experience in hospital, home health, or hospice care settings.
• Clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration and communication skills.
• Driver's license, car insurance, and reliable automobile for field work.

Preferred Skills & Certifications:

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
• Additional certifications relevant to specialty areas of nursing.

Special Considerations:

• Ability to work effectively in fast-paced and high-acuity environments.
• May require flexibility for on-call duties and varying patient care settings including hospice and home health.

Scheduling:

• Work schedule and shifts will be assigned based on facility needs and patient care requirements.
• May include day, evening, night, weekend, and holiday shifts.
