Location: Denver, CO, 80221
\n Float / Coverage Area: Local Travel required

Contract Length: 
\n Pay Range: $50.00 to $54.00/hour*\n
Mileage Reimbursement Rate (if applicable) : $0.51/ mile\n
*Pay offered is based on experience, expertise, credentialing, and education.

\n
• Perform comprehensive patient assessments in home health settings, including admissions, Start of Care (SOC), Recertifications, Resumption of Care (ROC), and Discharge visits.
\n
• Develop, implement, evaluate, and update individualized Plans of Care (POC) based on patient needs and physician orders.
\n
• Coordinate care with physicians, therapists, caregivers, referral sources, and interdisciplinary team members.
\n
• Provide skilled nursing services including medication administration, wound care, disease management, IV therapy, catheter care, venipuncture, and patient education.
\n
• Assess and manage pain, monitor patient progress, and identify changes in patient conditions.
\n
• Educate patients and caregivers regarding medications, home safety, disease processes, treatment plans, and self-care practices.
\n
• Supervise Home Health Aides (HHA), LVNs/LPNs, and other support staff as appropriate.
\n
• Ensure timely and accurate clinical documentation utilizing Homecare Homebase (HCHB) .
\n

\n
• Active and unrestricted Registered Nurse (RN) License in the practicing state.
\n
• Graduate of an accredited Associate Degree in Nursing (ADN) program or Nursing Diploma program.
\n
• Minimum 1-2 years of clinical nursing experience .
\n
• Minimum 2 years of Home Health experience .
\n
• OASIS assessment experience required.
\n
• Homecare Homebase (HCHB) experience required.
\n
• Current CPR Certification.
\n
• Valid driver's license.
\n
