Physical Therapist - Home Health (Travel)

Provide skilled physical therapy services in patients’ homes in the Farmington Hills, MI area. Conduct evaluations, develop individualized plans of care, deliver therapeutic interventions including gait and transfer training, and educate patients and caregivers on home exercise programs and safety. Document evaluations, progress, and outcomes in compliance with Medicare and regulatory requirements and collaborate with interdisciplinary team members to coordinate care.

Minimum Qualifications

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Active Michigan physical therapy license required to start the assignment.
• 1-2 years of home health physical therapy experience preferred; experience in home health required per client notes.
• Experience with HCHB electronic medical record system preferred.
• Proficiency with OASIS documentation and Medicare compliance.
• BLS certification required.
• Ability to work independently in the field and manage schedules and geographic territories.

Preferred Qualifications

• Strong clinical assessment and decision-making skills.
• Experience with mobility, balance, gait training, and fall prevention interventions.
• Effective interdisciplinary communication and care coordination skills.
• Time, territory, and productivity management experience.

Assignment Details

• Shift hours: Monday–Friday, 8:00 AM–4:30 PM.
• Weekly hour guarantee: 32 hours.
• Coverage area includes Wayne and Oakland counties, with additional patients in Macomb, Washtenaw, and Livingston counties; clinicians expected to travel where patient needs require.
• Mileage reimbursement between visits at $0.49 per mile.
• Travel from home to home for visits and management of schedules and territories required.

Impacting Patient Care

Deliver home-based rehabilitation that improves patient function and safety. Perform comprehensive evaluations, set measurable functional goals, and adjust plans of care based on patient progress. Communicate with physicians, nurses, and other therapists to ensure coordinated care. Recommend assistive devices and environmental modifications to support independence.
