Field Medical Case Manager: Coordinated Recovery
ForzaCare
ForzaCare is hiring a Field Medical Case Manager to lead care coordination for injured workers. You will connect workers, providers, employers, and insurers to ensure timely, transparent care and successful return-to-work outcomes.
This role emphasizes licensure as an RN or CRC, with valid credentials and state compliance. Travel to appointments is expected; CCM/CIRS credentials are highly valued and prior workers' compensation experience is strongly preferred.
