RN Clinical Manager Registered Nurse Home Health
RN Clinical Manager Home Health Services Full Time Location: Rock Hill Are you a passionate nursing leader ready to make a meaningful impact in home health? We're seeking a Registered Nurse (RN) Clinical Manager to oversee clinical operations and patient care services in our home health division. Key Responsibilities:
- Provide daily clinical oversight of patient care services.
- Coordinate referrals and admissions with internal and external sources.
- Evaluate patient needs and agency capabilities before admission.
- Supervise care planning, partner assignments, and care delivery.
- Ensure compliance with regulatory standards and agency policies.
- Mentor and coach clinical staff to promote morale and retention.
- Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.

Required: Active, unrestricted RN license in the state of practice. Minimum of two years RN experience (preferred). At least one year of Home Health experience (preferred). Current CPR certification.
