Therapist - Physical Therapist

Job Description

Job Summary:

The Physical Therapist is responsible for developing and implementing individualized treatment plans to improve patient mobility and function following illness or injury. They evaluate patients, deliver evidence-based interventions, and monitor progress to help clients achieve optimal recovery within their capabilities.

Responsibilities:

• Assess patients' physical condition and establish treatment goals based on medical prescriptions and clinical observations.
• Deliver targeted therapeutic interventions to promote recovery and minimize disability.
• Supervise Physical Therapy Assistants and coordinate care within the interdisciplinary team.
• Complete comprehensive documentation of clinical findings, treatments, goals, and discharge plans.
• Communicate patient progress and concerns to the Clinical Team Manager and ensure compliance with all documentation timelines.
• Perform initial evaluations, patient admissions, and re-evaluations as required.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Possess at least one year of Home Health experience.
• Demonstrate strong clinical assessment and treatment skills in a home health setting.
• Exhibit excellent communication and documentation abilities.
• Ability to work independently and collaboratively within a team.
• Maintain current CPR certification.
