Hospice Registered Nurse (RN) Case ManagerTrustbridge Hospice, member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life's most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care.
