Perform OASIS data collection and documentation, ensuring accuracy and timely submission. Provide comprehensive case management, coordinating services, referrals, and equipment as needed to support patient goals. Monitor patient status, adjust plans as necessary, and communicate changes to the care team and supervising clinician. Deliver patient and caregiver education on disease management, medications, safety, and discharge planning. Maintain high-quality documentation and maintain productivity targets, typically serving a high-volume caseload with thorough documentation.
