Care Coordination Supervisor

Job Description The Care Coordinator Supervisor is responsible for supervising the Care Coordinator RN positions, Care Coordinator Social Work positions, and the Navigator positions. The Supervisor will work collaboratively with their team as well as with providers as they provide medical, behavioral, psycho/social care, coordinating long-term services and supports, home and community-based services, health and wellness classes, and other related services and supports to members. The Supervisor serves as an active advocate for the members’ rights and plays a key role in the interdisciplinary care team and is a resource to the Director of Care Management, members, their families, and support systems. This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Travel is required and the percentage of travel is based on patient and enrollment needs.
