Primary Care Physician – Value-Based Care
Richmond, VA – Chamberlayne Area

A well-established, value-based primary care organization is seeking a Primary Care Physician for its Chamberlayne clinic in Richmond, Virginia. This opportunity is ideal for a physician who wants a more patient-centered model, smaller daily patient volume, strong clinical support, and a Monday–Friday schedule with no nights or weekends.

This clinic is located in the Richmond metro area and serves an adult and senior patient population with a focus on preventive care, chronic disease management, care coordination, and long-term patient relationships.

Responsibilities

• Provide comprehensive outpatient primary care to adult and senior patients
• Manage chronic conditions including diabetes, hypertension, COPD, CHF, and other common primary care concerns
• Emphasize preventive medicine, wellness, and long-term care planning
• Collaborate with an interdisciplinary care team to improve outcomes and reduce unnecessary hospitalizations
• Document patient encounters accurately and efficiently
• Build meaningful relationships with patients in a supportive, team-based clinical environment

Candidate Qualifications

• MD or DO degree
• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
• Active or eligible Virginia medical license
• Strong interest in outpatient primary care and value-based medicine
• Comfortable caring for adult and senior patients
• Excellent communication and bedside manner
