Waiver Support Coordinator/Case Manager

Job Description

Dynamic agency seeking Support Coordinator to provide case management services to individuals with developmental disabilities. Waiver Support Coordination includes ongoing case management to ensure that clients with developmental and intellectual disabilities access services needed to maintain their health, safety, and well-being. WSCs are responsible for assisting individuals in accessing services from all available resources and monitoring the effectiveness of home and community-based services. They advocate for individuals and work to identify, develop, coordinate, and access supports on the client’s behalf. Anchor Support Coordination Services offers a flexible schedule and a great working atmosphere. Must have a minimum of Bachelor's Degree, reliable transportation and the ability to work independently.

A bachelor’s degree from an accredited college or university.
Two years of paid, supervised experience in developmental disabilities, special education, mental health, counseling, guidance, social work, or health and rehabilitative services. A master’s degree in a related field can substitute for one year of the required experience.
Must have excellent problem solving skills, great communication, writing and math skills and computer literacy
