LPN Shift Options: 7a to 7p and 7p to 7a

Woods at Sparta is hiring Licensed Practical Nurses for 12-hour day and night shift openings. If you’re looking for a steady nursing role where you can build relationships with residents, support a consistent team, and make a real impact in skilled nursing, we’d love to connect.

As an LPN, you’ll provide hands-on resident care, administer medications and treatments, document clinical updates, monitor changes in condition, and communicate with RNs, CNAs, and nursing leadership.

Major Duties and Responsibilities 

• Directs the daily activities of the licensed practical/vocational nurse and certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures. 
• Follows the nursing model to promote holistic care for the residents. 
• Ensures that policies and procedures are complied with by nursing personnel assigned. 
• Participates in the admission, discharge and transfers of residents as required. 
• Assesses for changes in residents’ status, notifying the physician and resident’s family or representative and documenting accordingly. 
• Transcribes physician orders to medical record and carries out orders as written. 

Required Qualifications 

• Prior experience in long-term care preferred but not required. 
• A Nursing degree from an accredited college or university. 
• Current unrestricted license as a Registered Nurse (RN) in practicing state. 
• Current CPR certification.
