Step beyond the bedside and experience the rewarding nursing specialty of Home Health Case Management as a hybrid remote long-term home health case manager serving the Denver Metro Area. LiliSare supplies the equipment you need to be successful including hourly pay, Remote access, computer, technology package and nursing equipment with PPE. Schedule patients to be seen in their home with paid travel time and mileage reimbursement. Travel is required 40% of the time and the majority of the Case Manager responsibilities are completed from home via electronic medical record and other remote communication allows for a flexible hybrid work-from-home schedule! Responsibilities :

• Assessment of needs, development and implementation of plan of care and care coordination
• Complete documentation as required using the EMR Reviews clinical documentation to ensure its completeness
• Communication and collaboration with multiple disciplines and supervisor

Requirements:

• Active Colorado RN license
• Basic Life Support (AHA accredited)
• 1 year bedside nursing or 1 year Case Management experience
• COVID-19 vaccination or waiver required
• Reliable transportation and valid vehicle insurance is required (for travel to patients)
• Internet access and phone service (phone reimbursement available)

Skills:

• Analytical thinking, Clinical decision making and Problem solving.
• Ability to prioritize and organize workload independently
• Strong written and Oral communication abilities
• Electronic charting and basic computer skills - excel, Word and pdf

Experience:

• Nursing: 1 year (Preferred)
• Managed Care, Case Management or Leadership: 1 year (Preferred)

License/Certification:

• Colorado RN license (Required)
