Licensed Practical Nurse In Long Term Care

The Licensed Practical Nurse in Long Term Care performs total patient care services as directed. Measures, evaluates, administers and documents patient care. Contributes to patient care plans and informs Registered Nurse when appropriate. Provides direction and assigns duties to Certified Nursing Assistants in accordance with their skills and abilities. Actively responds to patient, physician and family requests. Advocates and ensures Patient Rights and Patient Privacy are upheld. SHIFTS: Sunday Night, and then possibly a run of either Thursday, Friday, or Saturday night shifts. We also need Tuesday / Wednesday Day shift but Sunday night is the big need.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Medication administration, care management and gives direct patient care, such as bathing, dressing, and feeding patients, and assisting in examinations and treatments.
• Ability to lift, walk, carry and stand.

SKILLS AND ABILITIES REQUIRED:

• Oversees total patient care and works directly with other staff to assure provision of highest standards of care.
• Compliance with the State of Nevada's Nurse Practice Act, Hospital policies and rules/regulations.
• Ability to organize and prioritize work to maintain harmonious daily workflow.
• Check, pour, administer and document medications at appropriate times. Monitors effects and side effects of medications.
• Works with intravenous medications and administer all routes of medications per Nevada State Board of Nursing criteria.
• Administers and records treatments as ordered. Monitors progress toward expected outcomes of treatments.
• Admits, transfers and discharge patients as required.
• Reports changes in condition, laboratory results, unexpected occurrences to the RN and the physician.
• Accurate documentation, reporting and in-depth nursing observation to assure continuity of patient care. Every shift charting, summarizing and updating patient care plans as needed.
• Daily evaluates patient care needs and condition changes, and implements changes in care delivery as warranted. Consults with the RN in implementing such changes.
• Assures that Patient Rights are upheld.
• Transcribes physician orders and communicates changes to other members of the team.
• Assures that Labs, X-rays, etc., are completed and notifies attending physician of abnormal results.
• Assists as necessary with patient care.
• Maintains cleanliness and order on unit.
• Participates in and satisfactorily complete the Orientation Program and Annual Review Days.
• Attends inservices and meetings as required.
• Pursues thirty (30) contact hours of continuous education, as required by the State of Nevada for re-licensure.
• Reports deviations in quality of care to Supervisors as outlined by current nursing standards.
• Actively participates in Quality Improvement activities.
• Maintains working knowledge of hospital-wide order entry.

EDUCATION AND EXPERIENCE REQUIRED:

• Graduation from a recognized Practical Nurse Program.
• Current licensure by the Nevada State Board of Nursing as an LPN.
• IV certification within one year of employment.
• Neat, well-groomed, professional appearance.
• Capable both physically and mentally for close work with elderly patients, their families, nursing and medical staff.
• Understands the need for confidentiality of information gained while performing the job. Performs other reasonable related duties as assigned by immediate supervisor and other management as required.
