Physical Therapist - Home Health

Location: Wood County, WV

Assignment Duration: This is a 13-week contract position.

Schedule: Monday through Friday, 8:00 AM - 4:30 PM

Job Description: The Physical Therapist will provide care to patients in their homes, ensuring excellent rehabilitation practices are followed. The role involves assessing patient needs, developing treatment plans, and implementing rehabilitation protocols for a variety of diagnoses.

Responsibilities:

• Conduct evaluations and create individualized treatment plans.
• Provide therapy interventions focusing on general orthopedic conditions, generalized weakness, and cardiac rehabilitation.
• Travel to patients' homes within Wood County, providing care in a home health setting.
• Maintain accurate and up-to-date patient records using the EPIC EMR system.
• Collaborate with other healthcare professionals to enhance patient care.

Required Experience / Certifications / Licensure:

• Current Physical Therapy licensure in the state of West Virginia.
• BLS Certification.
• Familiarity with EMR systems, preferably EPIC.
• Experience in a home health or outpatient setting preferred.
