Location Buckeye, AZ 85326 | 33.403271562 | -112.54866395 Job Summary Per‑Diem, Part‑Time, or Full‑Time Physical Therapist positions are available in the Southwest Valley (Laveen & Buckeye). BAYADA Home Health Care seeks an experienced Physical Therapist to conduct home‑health visits in the North Valley from our Glendale office, providing rehabilitative care and skilled nursing to adult and geriatric clients throughout the greater Phoenix area. Responsibilities Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as mobility and range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continuously assess and revise the Physical Therapy care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate. Educate and instruct clients, family members, or other representatives on rehabilitative care and activities necessary to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations related to client care management and services provided, utilizing a state‑of‑the‑art touch‑pad tablet. Qualifications A current Arizona Physical Therapist license. At least one year of recent work experience as a licensed Physical Therapist. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as shown by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills.
