NURSE PRACTITIONER I - Palliative Medicine (Outpatient)

Job Description Johns Hopkins Hospital is seeking a dedicated and experienced Nurse Practitioner to join its Palliative Medicine team in a primarily outpatient capacity. This role supports the expansion of outpatient palliative care services and offers an opportunity to provide comprehensive, patient‑centered care within a highly collaborative academic medical environment. Key Responsibilities Conduct comprehensive palliative care assessments and develop individualized care plans. Provide symptom management, goals‑of‑care discussions, advance care planning, and psychosocial support for patients with serious illness. Manage a panel of patients independently in both stand‑alone and embedded outpatient clinic settings. Collaborate closely with an interdisciplinary team, including physicians, social workers, nurses, pharmacists, and chaplaincy. Coordinate care with specialty services (e.g., oncology, cardiology, neurology) and community partners, including hospice agencies. Communicate effectively with patients, families, and referring providers to support shared decision‑making. Participate in interdisciplinary meetings, case discussions, and quality improvement initiatives. Schedule Monday–Friday, full‑time Majority of clinical time will be based on the East Baltimore / JHH main campus Primarily outpatient No call Qualifications Master’s degree in Nursing from an accredited program. Current Maryland Nurse Practitioner license. Certification as a Nurse Practitioner in a relevant specialty Strong clinical assessment skills and experience in palliative care, oncology, or geriatrics preferred. Salary Range Minimum 53.15/hour - Maximum 82.38/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
