Compassionate Hospice RN Case Manager

Taleo

Elite Hospice, part of LHC Group, is seeking a Registered Nurse Case Manager to lead the Plan of Care for hospice patients and collaborate with the care team to ensure compassionate, evidence-based nursing care across settings. You will evaluate eligibility within 48 hours, develop and implement care plans, and document patient progress while coordinating with physicians and the inter-disciplinary group to support patients and families. #J-18808-Ljbffr Taleo
