Licensed Practical Nurse - LPN
Bartlesville Health & Rehab Community is hiring Licensed Practical Nurses to join our nursing team. We are looking for dependable, compassionate, and clinically focused nurses who are committed to providing safe, respectful, and high-quality care to residents. LPNs provide nursing care in a skilled nursing and long-term care setting under the direction of nursing leadership and in accordance with physician orders, facility policy, and Oklahoma licensure requirements. This includes administering medications, completing treatments, monitoring resident condition, communicating with providers and families, documenting care, responding to changes in condition, and helping coordinate daily resident care. LPNs also provide direction and support to CMAs and CNAs, helping ensure resident needs are met timely and safely. This position plays an important role in medication safety, treatment accuracy, documentation, resident comfort, infection control, fall prevention, skin monitoring, and overall quality of care. A successful LPN is organized, calm under pressure, dependable, observant, professional, and able to communicate well with residents, families, providers, nursing staff, and facility leadership. - Be in good standing with the applicable nursing board or regulatory authority.
- Maintain current licensure, required continuing education, competencies, and facility education.
- Demonstrate knowledge of nursing care, medication administration, treatment administration, resident rights, documentation, infection control, and long-term care expectations.
- Demonstrate the ability to supervise CNAs and CMAs and communicate resident care needs clearly.
- Be able to read, write, speak, and understand English sufficiently to perform nursing duties, understand orders, complete documentation, and communicate resident needs.
- Maintain current CPR certification if required by facility policy or assigned duties.
- Pass required background checks and meet employment eligibility requirements.
- Previous LPN experience in skilled nursing, long-term care, assisted living, memory care, hospital, home health, hospice, rehabilitation, or a similar healthcare setting.
- Experience supervising CNAs and CMAs.
- Experience with electronic health records, electronic medication administration records, and electronic treatment records.
- Experience with wound care, skilled documentation, Medicare, Medicaid, managed care, hospice, dementia care, behavioral symptoms, infection control, and resident assessment.
- Experience communicating with physicians, nurse practitioners, families, pharmacies, therapy staff, and interdisciplinary team members.
- Provide safe, competent, compassionate, and resident-centered nursing care.
- Administer medications and treatments accurately and safely.
- Recognize and respond to resident changes in condition promptly.
- Communicate clearly and professionally with residents, families, providers, coworkers, CNAs, CMAs, nurses, and leadership.
- Supervise, direct, support, and hold assigned nursing assistants and medication aides accountable.
- Complete documentation accurately, objectively, and timely.
- Follow physician orders, care plans, policies, nursing standards, infection control expectations, and resident rights requirements.
- Maintain confidentiality and professional boundaries.
- Remain calm and professional during emergencies, resident distress, family concerns, staffing challenges, behavior symptoms, and busy periods.
- Organize and prioritize multiple resident needs, medications, treatments, documentation requirements, and staff questions during the shift.
- Work cooperatively with nursing, therapy, dietary, activities, social services, housekeeping, laundry, maintenance, admissions, MDS, business office, and administration.
- Use sound judgment and request guidance when orders, resident condition, documentation, reporting requirements, staffing concerns, or scope of practice are unclear.
- Stand, walk, sit, bend, stoop, kneel, reach, push, pull, lift, carry, and move throughout the workday.
- Push and maneuver medication carts, treatment carts, wheelchairs, beds, lifts, resident care equipment, and supplies.
- Assist residents with transfers, repositioning, walking, toileting, bathing, dressing, grooming, meals, hydration, and other care needs when necessary.
- Lift, move, and position residents using proper body mechanics and facility-approved equipment.
- Perform repetitive motions related to medication administration, treatment administration, documentation, resident care, and equipment use.
- See, hear, communicate, and respond to resident needs, medication labels, physician orders, alarms, call lights, staff communication, and environmental concerns.
- Respond quickly to resident distress, falls, choking concerns, medication reactions, behavioral symptoms, or emergency situations.
- Work assigned shifts, including weekends, holidays, evenings, nights, overtime, on-call needs, or adjusted schedules as required by facility operations.
The Licensed Practical Nurse must be able to safely perform essential job functions with or without reasonable accommodation. Work Environment The Licensed Practical Nurse works in a skilled nursing and long-term care environment. This position involves frequent interaction with residents, families, nursing staff, CNAs, CMAs, therapy staff, dietary staff, activities staff, social services, housekeeping, laundry, maintenance, physicians, nurse practitioners, pharmacists, hospice staff, visitors, surveyors, and other facility personnel. The LPN may be exposed to medications, infectious materials, bodily fluids, cleaning chemicals, odors, noise, emotional situations, behavioral symptoms, resident emergencies, end-of-life situations, family concerns, and physically demanding care tasks. Proper medication safety, infection control, resident rights, confidentiality, and professional conduct are required at all times. Nursing Care and Supervision Responsibility The Licensed Practical Nurse is responsible for providing assigned nursing care and supervising assigned CNAs and CMAs during the shift. The LPN must ensure resident needs are addressed, care is provided according to the care plan, medications and treatments are completed as ordered, and resident concerns are reported and followed up appropriately. The LPN must communicate clearly with nursing leadership regarding resident condition changes, staffing concerns, care concerns, medication concerns, documentation concerns, or other issues affecting resident safety or quality of care. Medication and Treatment Responsibility The Licensed Practical Nurse is responsible for administering medications and treatments safely, accurately, and according to physician orders and facility policy. The LPN must follow medication rights, treatment procedures, infection control practices, documentation requirements, and pharmacy procedures. The LPN may not administer medications or treatments outside scope, change orders without proper authorization, borrow medications from another resident, leave medications unsecured, falsify medication or treatment documentation, or fail to report medication and treatment concerns promptly. Documentation Responsibility The Licensed Practical Nurse is responsible for completing nursing documentation accurately and timely according to facility policy. Documentation may include medication administration, treatment administration, progress notes, change of condition notes, incident documentation, physician notifications, family notifications, skilled charting, wound documentation, behavior documentation, refusal documentation, and other assigned records. The LPN may not document care that was not provided, falsify records, document inaccurate information, omit required notifications, or fail to report resident concerns when nurse follow-up is required. Resident Rights and Professional Conduct The Licensed Practical Nurse is expected to uphold resident rights at all times. This includes respecting resident dignity, privacy, choices, culture, preferences, routines, personal property, relationships, care decisions, medication rights, visitors, and the right to be free from abuse, neglect, exploitation, discrimination, retaliation, and unnecessary restraints. The Licensed Practical Nurse must maintain a professional attitude and may not engage in rough treatment, disrespectful language, intimidation, retaliation, favoritism, gossip, unauthorized disclosure of confidential information, inappropriate social media use, unauthorized photography, falsification of records, misleading statements, medication diversion, neglect of resident needs, or any conduct that compromises resident dignity, safety, privacy, quality of care, or trust. Attendance and Reliability Because nursing care, medication administration, treatment administration, resident supervision, documentation, staff direction, emergency response, and shift communication are essential to resident safety and quality of care, regular attendance and punctuality are essential functions of this position. The Licensed Practical Nurse is expected to report to work as scheduled, complete assigned duties, communicate resident concerns promptly, and follow facility call-in and attendance procedures. Scope of Practice The Licensed Practical Nurse may perform duties consistent with licensure, training, competency, facility policy, physician orders, nurse supervision requirements when applicable, and applicable requirements. The LPN may not perform duties outside LPN scope, independent medical diagnosis, independent prescribing, clinical duties requiring RN licensure unless permitted by law, therapy duties, social services duties requiring licensure, or other tasks outside authorized scope. The LPN must seek guidance from the RN Supervisor, Director of Nursing, Assistant Director of Nursing, Administrator, physician, nurse practitioner, pharmacist, consultant, or appropriate supervisor when orders, resident needs, clinical condition, documentation, reporting requirements, staffing concerns, or scope of practice are unclear.
