Job Summary:

Tampico Healthcare Center, a 128-bed skilled nursing rehabilitation center located in the heart of Walnut Creek, CA, is seeking a full-time Occupational Therapist to join our in-house therapy team! As a key member of our multidisciplinary team, you will have the opportunity to work with patients who require short-term rehabilitation therapy to longer-term restorative care.

Responsibilities:

• Evaluate patients in a timely manner following physician referral
• Develop effective treatment plans and provide education for clients, families, and caregivers as a subject matter expert
• Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans
• Participate in inservice training programs for other staff in the facility
• Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy
• Provide accurate and timely billing records and supporting clinical documentation
• Instruct patients' families or nursing staff in maintenance programs
• Coordinate necessary durable medical equipment and adaptive equipment for patients to facilitate safety and independence in ADL's
• Ensure resident abuse or neglect does not occur
• Observe confidentiality and safeguards for all patient information
• Comply with federal, state, and local legal and professional requirements
