Physical Therapist

Position: Physical Therapist Salary Range: $39-45/hour Schedule: 35 hours per week, Monday-Friday, no weekend rotation Facility: Skilled Nursing Facility (SNF) with short term and long term rehabilitation services, as well as a 17 bed assisted living facility (ALF) Services: Offering occupational therapy (OT), physical therapy (PT), and speech language pathology (SLP) services to patients in both the SNF and ALF settings. Responsibilities Conduct thorough evaluations of patients to determine their physical therapy needs and develop individualized treatment plans. Provide hands‑on therapy to patients, including exercises, manual therapy, and other modalities as needed. Monitor and document patient progress, adjusting treatment plans as necessary. Communicate with other healthcare professionals, including occupational therapists and speech language pathologists, to coordinate patient care. Educate patients and their families on home exercise programs and techniques to improve overall function and mobility. Maintain accurate and timely documentation of patient progress and treatment plans. Collaborate with interdisciplinary team members to ensure comprehensive and effective patient care. Qualifications Must hold a valid Physical Therapy license in the state of [state]. Minimum of 1 year experience as a Physical Therapist in a SNF setting preferred. Strong communication and interpersonal skills. Ability to work independently as well as part of a team. Excellent time management and organizational skills.
