Volunteer AmeriCorps Patient Health Navigator

Position Summary Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC), is recruiting for a Sea Mar – AmeriCorps Member volunteer position. This full‑time volunteer role includes a modest living allowance stipend and AmeriCorps service benefits. Responsibilities Schedule appointments and guide patients through health system barriers. Initiate communication with patients, assist with referrals, and ensure pre‑ and post‑visit needs are met. Participate in mandatory trainings, service projects, and team‑building activities during the 10‑11 month term. Create informational materials, posters, handouts, and community resource guides for Sea Mar sites. Review electronic health record charts to identify patients in need of screenings and services; schedule appropriate appointments. Collaborate with medical clinic staff, quality improvement staff, and other departments to promote preventive care and outreach efforts. Lead community outreach and health events on topics such as smoking cessation, diabetes education, nutrition, and chronic condition management. Aid in public awareness of health coverage enrollment opportunities through outreach activities and events. Develop and distribute informational/promotional materials and maintain referral relationships with local organizations. Qualifications Bilingual English/Spanish preferred. Strong written and verbal communication skills. Ability to work independently, use good judgment, meet deadlines, and prioritize tasks. Proficiency in Microsoft Office and basic computer skills. Good organizational skills, attention to detail, and professionalism. Valid background check and current TB test (within the past year). High school diploma required; preferred undergraduate degree or equivalent work experience. Reliable transportation. Benefits and Compensation AmeriCorps living allowance stipend up to $20,400 prorated over 1,700 hours (service ends August2027). Post‑service education award up to $7,395.
