Physical Therapist (PT) / Occupational Therapist (OT)

Location: Dousman, Wisconsin
Employment Type: Full-Time
Setting: Skilled Nursing, Assisted Living, Independent Living, and Memory Care Communities
Schedule: Monday–Friday (Day Shift) with one weekend rotation per month

Position Summary

The Physical Therapist or Occupational Therapist will provide comprehensive rehabilitation services to residents across multiple levels of care. This role offers an opportunity to work with a diverse patient population while promoting functional independence and improving quality of life.

Responsibilities

• Perform comprehensive evaluations, reassessments, and treatment planning.
• Deliver individualized therapy interventions based on patient goals and clinical needs.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Maintain thorough and accurate clinical documentation.
• Participate in care conferences and discharge planning as needed.
• Ensure compliance with state, federal, and organizational guidelines.

Qualifications

• Active Wisconsin Physical Therapist or Occupational Therapist license.
• Excellent clinical, organizational, and communication skills.
• Ability to manage a varied caseload effectively.
• Open to both experienced therapists and new graduates.
