Physical Therapist, (PT) - Home Health

$2,500 Sign On Bonus! Bayada Home Health Care is seeking an experienced Physical Therapist for a full‑time home health position in our Pitt County office. This role provides home health services to adult and geriatric clients across Pitt and Wilson counties. As part of a multidisciplinary team, you'll deliver rehabilitative care and skilled nursing to help clients stay at home. Responsibilities Make home visits to clients in designated geographic territories. Perform diagnostic tests and measurements, such as the mobility, range of joints, transfer status, stability, patterns and appearance of ambulation, strength, endurance of muscles, balance testing, and safety assessments. Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continuously assess and revise the Physical Therapy care plan, and collaborate with nursing in the multidisciplinary care plan as appropriate. Educate and instruct clients, family members, or other representatives on rehabilitative care and activities necessary to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet. Qualifications A current North Carolina Physical Therapist license. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills.
