Physical Therapist

Join as a licensed Physical Therapist dedicated to delivering personalized, one-on-one care to older adults in their homes, senior living communities, or through virtual platforms. Key Responsibilities Provide individualized physical therapy to enhance mobility, independence, and quality of life. Perform patient evaluations, develop care plans, and implement evidence-based treatments. Document patient progress accurately within electronic medical records. Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Utilize telehealth and virtual care to extend reach and flexibility. Qualifications And Skills Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited Physical Therapy program. Proficiency in computer use and electronic medical records. Current CPR certification. Physical requirements: Ability to perform moderate physical effort, assist with patient transfers, stand and walk for extended periods, and drive to patient locations. Must be able to wear PPE as needed.
