Behavioral Health Consultant

Summary: The School Based Health Center (SBHC) Behavioral Health Consultant (BHC) position requires an independent license such as an LCSW (licensed clinical social worker) or LCPC (licensed clinical professional counselor) to practice in a school-based and community health care setting. This is a 12-month position, 10 months at an SBHC, and 2 months working at a community health center during summer break. The SBHC BHC is assigned to one of Tapestry 360 Health’s SBHC’s during the school year, and provides students with counseling, groups, screenings, and risk assessments. During summer break, the SBHC BHC will be assigned to a community health center to provide patients with counseling, groups and screenings. The individual in this position must be comfortable providing in-person and telehealth counseling, working in a fast-paced environment, with a flexible schedule, able to complete at least 8 visits per day, and adept in working closely with medical providers, school staff and other care team members. Other required skills and duties are outlined below.Essential Duties and Responsibilities:Provides direct student/patient care individually or in groups, in person or via telehealth, assisting students/patients achieve therapeutic goals in accordance with their treatment plan, and coordinates care to improve students'/patients' overall wellbeing via 30 minute counseling appointments.See students/patients immediately after their appointment with their medical provider through warm hand-offs and develop a plan of action to address the presenting need.Work as part of an interdisciplinary team to assist and collaborate with educators and medical providers in recognizing students at risk, treat students’ behavioral health and psychological disorders and provide the students’ care team with timely feedback about the students’ care, treatment recommendations and consultations.Provide direct patient care during summer break individually or in groups, in person or via telehealth, assisting patients achieve therapeutic goals in accordance with their treatment plan, and coordinates care to improve patient outcomes via 30-minute, brief targeted interventions, or short-term counseling and/or screenings.Ability to work effectively with a diverse student/patient and staff population and use of interpreter services as needed.Basic understanding of primary care and nontechnical knowledge of psychotropics and other relevant medications.Provide referrals/resource information on care coordination and access to external programs for students/patients as needed.Develops and maintains relationships with external providers, staff, and programs.Attends, participates in and reports on various internal and external meetings and training sessions.Excellent computer skills and the ability to work in an electronic health record. Maintains documentation of the patient’s treatment by using current and accurate ICD-Coding and CPT coding.Establishes and maintains a professional and therapeutic relationship within the standards of patient profiles, confidentiality laws and the newest clinical research available.Attend trainings, workshops, and other relevant professional development to maintain licensure.Adheres to professional standards as outlined by governmental bodies, NASW (and/or other appropriate professional associations), private funding sources, organization plans/policies and organization’s guidelines. Ability to deal with problems involving several concrete variables in standardized situations.Competencies: Ability to use computers to receive, organize, prioritize, and transmit information. Proficiency in use of Microsoft Office suite including Outlook, Word, Teams, and Excel. Comprehension of and appropriate use of basic medical terminology, application of third-party payer guidelines and concepts related to quality assurance measures.
