Medical Social Worker

Assignment Duration

This is a 13-week contract position.

Schedule

Day shift, 5x8 hours (08:00 - 16:30). Required to work every other weekend with holiday coverage.

Job Description

The Medical Social Worker will provide support to patients within an acute care setting, focusing primarily on discharge planning and care coordination. The role involves interacting with patients from various demographics, ensuring they receive appropriate resources and support as they transition from hospital to home care.

Responsibilities

• Conduct biopsychosocial assessments to determine patient needs.
• Collaborate with multidisciplinary teams for comprehensive patient care.
• Facilitate discharge planning and contingency planning.
• Educate patients and caregivers about resources and care options available.
• Document patient interactions and care plans accurately in the electronic health record system (Epic).
• Manage cases for patients experiencing homelessness, mental health challenges, or substance abuse issues.
• Assist in the navigation of the Department of Children and Families (DCF) process.

Required Experience / Certifications / Licensure

• A minimum of 2 years of acute hospital case management experience required.
• Experience with discharge planning and care coordination.
• California state license preferred; pending licenses accepted.
• Certification in MSW (Master of Social Work) or equivalent is required.
