Registered Nurse – Home Health

Schedule: Monday–Friday, 8:30 AM–5:00 PM
On-Call: 6 days/month, including 2 weekend days and 4 weekdays
Productivity: 30 units per 40-hour week
Mileage: $0.43/mile

Service Area: Chatham, Liberty, Bryan & Effingham Counties, GA

Position Overview

We are seeking an experienced Registered Nurse (RN) to provide high-quality home health care, coordinate individualized care plans, and support positive patient outcomes throughout the community.

Responsibilities

Perform comprehensive patient assessments and develop individualized care plans.
Provide skilled nursing care and monitor patient progress and outcomes.
Coordinate care with physicians, social workers, discharge planners, families, and other healthcare professionals.
Manage care transitions and connect patients with appropriate support services.
Educate patients and families on treatment plans, disease management, and prevention.
Maintain accurate and timely clinical documentation.
Identify opportunities to prevent adverse outcomes and promote overall health.
Advocate for patient privacy, safety, and quality of care.

Requirements

Unencumbered Georgia RN License required.
Home health experience required.
Proficiency with OASIS documentation and accuracy .
Experience using HCHB (Home Care Home Base) .
Associate's or Bachelor's Degree in Nursing or related field.
Strong clinical judgment, communication, and organizational skills.
Case Management certification preferred.
