Care Manager

Enlightened is growing our Complex Care Management team and we're looking for licensed Registered Nurses (RN) and Clinical Social Workers (LICSW) who are passionate about making a real difference in the lives of our Medicaid members. This isn't a behind-the-desk role. You'll be out in the community, meeting face-to-face with enrollees, coordinating care across providers, and helping people with complex medical needs navigate a system that can feel overwhelming — all while building meaningful, ongoing relationships with the people you serve. What you'll do Conduct comprehensive, person-centered assessments across physical, behavioral, and social determinants of health. Develop individualized care plans. Coordinate services across PCPs, specialists, behavioral health providers, and community resources. Support medication adherence and help enrollees achieve their health goals. What we're looking for 3–5 years experience with Medicaid or complex care populations Background in complex care, case management, or community health Experience working with adults and/or pediatric populations across the lifespan DC Metro resident (DC residents strongly preferred) Bilingual English/Spanish a plus CCM or URAC-recognized certification a plus The details Washington, DC (SW office — within walking distance of two Metro stations) $98,000 – $108,000 annually If you're a clinician ready to go beyond the exam room and truly coordinate whole-person care for the people who need it most — we'd love to hear from you.
