Licensed Practical Nurse 12 hour shifts

Position Summary The Licensed Practical Nurse is responsible for providing quality nursing care to residents, coordinating total nursing care, supporting the Director of Nursing and Unit Managers in establishing goals, ensuring compliance with policies and regulations, supervising personnel, reporting, and performing duties of a Certified Nurse Aide when required. Responsibilities Assess resident conditions to implement interventions and develop Care Plans; accurately record observations and ensure medical records policies are followed. Make regular rounds independently and with a physician; observe and evaluate resident symptoms, progress, and response to treatments and medications, implementing interventions as necessary. Provide preventative, supportive, maintenance, and rehabilitative care for the physiologic and psychosocial needs of residents. Perform treatments including dressing changes, wound cleaning/irrigation, Foley catheter irrigation, IV therapy, tube feedings, chest tubes, and peritoneal dialysis. Request and ensure availability of supplies and medical equipment required for resident care. Schedule appointments, physicians, therapy, and consults; document in appointment book. Monitor and assist CNAs with personal resident care duties (bathing, feeding, ambulation) and routine services for resident comfort and well‑being. Receive report from outgoing nurse and conduct daily rounds to see each resident as needed. Administer medications according to legal, nursing policies, physician orders, and notify staff of contraindications. Advise residents and/or responsible persons on medication changes, treatment plans, and resident condition; manage and document emergency care and coordinate communications to physicians, DON, Administrator, hospital personnel, emergency transport, and appropriate staff. Perform comprehensive nursing assessments during condition changes and scheduled programs; admit, transfer, and discharge residents; assure supplies and equipment are available; complete weekly, periodic, and discharge summaries. Conduct quality improvement activities as directed by the Director of Nursing. Monitor accident/incidents and infection rates, and develop corrective action suggestions. Perform weekly skin assessment to monitor pressure injuries or other skin conditions; report findings to the Director of Nursing. Report resident and staff complaints, unusual occurrences, or allegations of abuse or neglect to the Administrator immediately. Participate in periodic regulatory surveys and help prepare Plan of Correction responses. Report resident injuries to the Unit Manager. Uphold residents’ rights, including no restraints and no abuse; report incidents promptly. Support facility philosophy, standards, policies, and procedures. Communicate with department heads as needed to solve problems and plan effective care; inform Director of Nursing when staffing requirements are not met. Maintain a professional appearance and serve as a unit role model. Fulfill continuing education and staff development requirements through seminars and related activities. Promote compliance with accident prevention, safety rules, and safe work practices to protect employee health and reduce worker’s compensation costs. Ensure safe equipment use and report maintenance concerns to the Environment Plant Manager; adhere to the Nurse Practice Act. Attend staff meetings and in‑service training as requested. Perform other tasks as required. Required/Desired Qualifications Current Michigan license as a Licensed Practical Nurse. CPR certification. Two years of professional nursing experience in long‑term care (preferred). Specific Skills, Knowledge, and Abilities Knowledge of current federal and state laws and regulations governing nursing practice in long‑term care. Strong organizational and time‑management skills, with ability to prioritize responsibilities. Ability to apply mathematics (fractions, percentages, ratios, proportions) to practical nursing situations. Basic computer proficiency. Other Special Requirements Tolerance for frequent exposure to blood, body tissues, fluids, hazardous materials, infectious diseases, chemicals, latex, and plastic used in personal protective equipment.
