This position is classified as Non-Exempt and reports to the Director of Rehabilitation and the Facility Administrator.Core responsibilities include:Conducting comprehensive occupational therapy evaluations and re-evaluationsDeveloping, implementing, and modifying individualized plans of careDelivering skilled therapeutic interventionsAssessing and recommending adaptive equipmentMonitoring patient progress and modifying treatment plansReviewing patient medical history and conducting patient interviewsEvaluating patient home or workplace environmentsLeading discharge planning and transition-of-care coordinationCommunicating evaluation findings and progress updatesEducating patients, families, employers, and caregiversSupervising and directing Occupational Therapist AssistantsProviding oversight to rehabilitation aides, students, and volunteersCompleting timely, accurate, and compliant documentationParticipating in care conferences, quality assurance initiatives, and collaborative planningMaintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable Arizona state practice standardsTreating all residents with dignity, respect, and compassionate professionalismPhysical requirements include the ability to assist with patient transfers, upper extremity handling and positioning techniques, mobility assistance, repositioning and supporting patients with or without assistive devices, prolonged standing, walking, bending, reaching, kneeling, and stair navigation.
