Love Where You Work.As a Clinical Home Care Coordinator, you will be responsible for the efficient and effective facilitation of referrals to the post-acute homecare/hospice agency once identified in the acute care setting; the provision of education to acute care staff, including physicians, regarding services available from homecare/hospice; and may also be involved in business development activities as they relate to business expansion or new program development.Why Join Us?Start Here… Grow Here... And your benefits begin on your first day!Supportive Leadership Our management team is here to help you succeed every step of the way.Epic EMR System Streamlined documentation and communication for better care and less stress.Fast Hiring Process Quick interviews and timely offers because your time deserves respect.What Will You DoMaintains daily contact with case managers/discharge planners and units to help and assure that patients referred for services are followed up in a timely and efficient manner.Interacts with healthcare team members to answer any questions related to homecare/hospice and community-based services as they plan for patient discharges from the acute care setting.Coordinates care for clients going home from the hospital who have chosen the agency's services.Reviews and gathers information from the hospital medical record, copying portions for transmission as needed.Documents initial assessment of homecare needs and potential plan for services in compliance with agency policy.Arrange service in accordance with the established admission criteria, client care needs and client preference.Obtains physician orders for initiation of homecare/hospice services in accordance with agency policy/procedure and third-party regulations.Utilizes interview, observation, and evaluation in assessing clients and applies clinical judgment, consistent with practice standards, in determining appropriate home care services and making recommendations to the physician and client.Benefit HighlightsMedical, dental, and vision coverage starting Day OneShort- and long-term disability403(b) retirement plan with employer matchGenerous paid time off plus 7 paid holidaysTuition reimbursement up to $5,250/yearMileage ReimbursementComprehensive onboarding and individualize orientation programMinimum QualificationsBachelor's degree or equivalent level of experience required.Graduate of an approved clinical education program and current clinical license in the state of employment preferred.Must have a minimum of two (2) years' experience in acute care setting, customer service, sale's/marketing role or relevant experience.Must have current Driver's license and reliable transportation to and from work site.Ability to consistently demonstrate a commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.Well-developed oral and written communication skills.About Trinity Health At Home – (Michigan)Join one of America's best home health agencies.Trinity Health At Home is a national home care, hospice and palliative care organization serving communities across 11 states.As a faith-based, not-for-profit agency, we are committed to caring for patients and families in the place where they feel most comfortable—at home.
