Key Responsibilities

• Performing thorough patient assessments, ordering appropriate tests, and developing personalized care strategies
• Diagnosing and managing acute, chronic, or episodic health conditions in tandem with physicians or independently (per your credentialing)
• Providing education to patients and families on preventive care, wellness, and effective disease management
• Prescribing medicines and therapies, adhering to your scope of practice
• Engaging in patient follow-ups and chronic care management assessments
• Working collaboratively with interdisciplinary teams for comprehensive, patient-focused care
• Meticulously documenting patient interactions within electronic medical records (EMR)

Qualifications

• Earned a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Licensure and Certifications

• Active Nurse Practitioner license in the state of Ohio
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or specialty equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to obtain within 90 days)
