RN - Case Manager

Registered Nurse Case Management

We are seeking a Registered Nurse for a Case Management position. The role is within a psych/rehabilitation department and requires flexibility as it is treated as a float unit. The nurse will provide case management services, focusing on complex discharge planning. The unit's patient-to-staff ratio is approximately 25:1 for case management. The staff consists of Registered Nurses and Social Workers. Experience with electronic medical record systems is essential for this role. We offer a collaborative work environment with seasoned staff members. Candidates must be open to working every other weekend and holidays in rotation with facility employees.
