Hartford Healthcare is seeking a Weekend On Call Visiting Nurse to provide high-quality care to home care patients across the community. As an RN, you will perform comprehensive assessments, deliver skilled nursing (wound care, IV therapy, medication management), and respond to weekend and after-hours calls. You’ll collaborate with an interdisciplinary team, update care plans in the EMR, educate patients and families, and ensure safe transitions of care.
