Job Summary The Practice Ambassador is a dynamic sales position focused on cultivating referral relationships and partnerships with assisted, memory care, independent living communities, skilled nursing facilities, hospitals, and other referral sources that serve our patient population. The role fosters referral relationships, educates potential partners and patients about our services, and advocates for the benefits of our home‑based primary care and hospice solutions, driving increased referral volume and census growth. This position plays a pivotal part in expanding access to high‑quality in‑home care while supporting the growth and success of our practice. Practice Ambassador Job Functions Assist in the design and implementation of Bloom’s market strategy. Oversee and manage all call points in the territory: assisted living facilities, skilled nursing facilities, hospitals, and external care and referral partners (home health, hospice, placement agencies, non‑medical, etc.). Obtain monthly, quarterly, and annual budget numbers and quotas to ensure organizational growth. Educate and meet face‑to‑face with patients and their families to assist them through the enrollment process. Practice Ambassador Requirements Prior sales or marketing experience in assisted living, hospital, home health, hospice, or a related industry is a must. College degree is preferred.
