Position: Physical Therapist Assistant (PTA)

Job Description

We are seeking a dedicated Physical Therapist Assistant (PTA) for a home health assignment. The role involves visiting patients in their homes, providing therapy and rehabilitation services, and documenting progress and care details.

Schedule and Work Environment

• Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)
• Contract Length: 13 weeks
• Average Number of Patients per Day: 5-6
• Locations: Primarily home visits in Griffin, GA, with potential assignments in Spalding, Lamar, Upson, Meriwether, and Pike counties.
• Travel: Must be willing to travel up to 75 miles from the assigned care center.
• Mileage Reimbursement: IRS Standard rate is provided for travel between patients' homes.

Key Responsibilities

• Conduct therapy visits and provide care as per the treatment plan.
• Document patient progress, treatment provided, and any changes in the patient's condition.
• Participate in periodic office meetings as scheduled.
• Accept every patient on the daily schedule and avoid cancellation unless absolutely necessary.

Qualifications

• Experience: Minimum of 1 year of adult home health experience is required.
• Certifications: Current CPR and BLS certifications.
• License: Must have a valid driver's license and agency-required liability insurance.
• EMR Proficiency: Experience with HomeCare HomeBase and OASIS is required.

Productivity Expectations

The expected weekly productivity is 30 points, equating to approximately 36 patients per week minimum. Each patient visit and related activities have different point values.

There are no on-call requirements for this position.
