Our Social Workers provide complex care management in the primary care setting. Through sensitivity to social drivers of health, understanding of the role behavioral health challenges often play in a patient\'s overall health, and deep knowledge of socioeconomic safety net systems and resources, the Social Worker helps patients overcome barriers and make meaningful progress toward health-related goals. Summary: The Social Worker – MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care. Required Minimum Knowledge, Skills, and Abilities (KSAs) Education: Bachelor\'s Degree in Social Work from an institution accredited by the Council on Social Work Education required. License/Certifications: Active license by the Maine State Board of Social Work required. Experience: Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required.
