Physician Assistant | Family Practice Role Overview: Providing primary care in family practice settings, including Home Based Primary Care and Assisted Living, with responsibilities focused on assessment and management of acute and chronic health issues for patients in skilled nursing, assisted living, and home-bound populations. Responsibilities Conduct patient evaluations. Manage health conditions. Coordinate with support staff. Work independently, traveling to patient homes utilizing the Cerner EMR system. Schedule flexibility includes 8, 10, or 12‑hour shifts, with 4x10 shifts and no on‑call duties. Qualifications & Skills At least 6 months of clinical experience. Preferably a background as CNA or RN in assisted living or nursing homes. New graduates are not accepted. Strong assessment skills. Ability to work solo. Familiarity with primary care.
