About the Position

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Physical Therapist Duration: 13 weeks Shift: Day Shift Hours per Shift: 8 hours Experience: One to two years related experience and/or training, or equivalent combination of education and experience. License: Current Physical Therapy license, or eligible for registration in the state. Certifications: Current BLS course completion. Must-Have: - Bachelors Degree from an accredited college or university in physical therapy, or degree in a related field. - Valid drivers license when travel is required. - Evidence of automobile insurance when travel is required. Description: Works under limited supervision while administering various standard physical therapy treatments for disabilities, injuries, and diseases. Aims to relieve pain, restore motor function, and maintain optimal performance. Job responsibilities include observing work schedule and job requirements. Maintains compliance with hospital and departmental policies including attendance and confidentiality. Completes patient evaluations within 24 hours of the physician order. Prepares discharge forms within 24 hours of discharge. Plans health care services based on physician orders and patient needs. Documents patient assessment and ongoing care according to agency policy and physical therapy standards. Encourages team members to promote patient and family independence. Ensures coordination of health care team services and community resources. Evaluates patient care needs on admission and on an ongoing basis. Instructs personnel in proper lifting techniques and use of lifting equipment. Maintains compliance with all competencies, license, certifications, and education requirements. Participates in Performance Improvement Plan activities as requested. Coordinates referrals to other health care team members based on physician orders. Communicates changes in patients' conditions and/or orders to team members. Supervises administration of treatments to patients by assistants and aides. Maintains physical therapy skills inventory list annually. Willingly performs other assigned duties and innovations. Charting is done through electronic medical record system while maintaining a manual log. Coverage is scheduled based on patient load and communicated during morning huddles. Weekend coverage needs are determined daily based on patient requirements.
