Home Health Physical Therapist, $20,000 Bonus

At Residential Home Health and Hospice (‘Residential’), we’re looking to add to our extraordinary care team. Grounded by our belief that outstanding care is best delivered in a team-based environment, our home health physical therapists collaborate to provide management and delivery of patient care plans. This full‑time position supports patients in Gurnee and surrounding areas and is eligible for $20,000 total bonus potential, including a $10,000 first‑year success bonus and an additional $10,000 sign‑on bonus paid within the first 18 months of employment. To receive these bonuses, you must be in good standing, meet performance expectations, and remain in the role and status for which you were hired.
