If so, we have an exciting opportunity for you to work as a Part Time (20 hrs/wk) Patient Navigator as part of our Mobile Crisis Response Team serving youth and adults. What You Will Be Doing Working as a Patient Navigator, you will be responsible for co-responding with clinical staff to provide mental health crisis intervention. This role involves responding directly to crisis calls in the community, including individuals homes, offering immediate support, de-escalation, and linkage to appropriate mental health and social services. You will be responsible for modeling a recovery-based lifestyle and sharing your personal story and experience to help people receiving services or experiencing mental illness achieve their own recovery. You will have the opportunity to provide input into policy development and help to assure consumer sensitive language and will participate on committees and in-service initiatives as requested. The individual in this position will be knowledgeable about and actively support 1) culturally competent, recovery based practices, 2) person centered planning as a shared decision making process with the individual, who defines his or her own life goals and is assisted in developing a unique path toward those goals and 3) a trauma informed culture to aid consumers in their recovery process. You will assist the team in providing comprehensive care for those receiving services. This position will have a typical shift schedule of Thursday and Friday 12 pm - 10 pm; with eligibility for weekend and evening shift premiums as applicable; quarterly holidays and shift variability may be discussed during the interview process.

We will be hiring one individual for this role to support our 24/7 Mobile Crisis Response Team. What We Require You will have a High School diploma or GED with at least one-year relevant work experience; experience in a mental health setting or Community Mental Health is preferred. You will have a knowledge of self-determination principles and person-centered planning process. Michigan Certified Peer Support Specialist designation strongly preferred, with ability to obtain certification required. You will also be comfortable utilizing basic computer skills, including word processing, spreadsheets, database management and e-mail and basic knowledge in the use of electronic office equipment, including copy and fax machines.

May encounter community environments that include homes, schools, courts, and other community settings that may or may not be barrier-free. Travel throughout the Kalamazoo area may be required.
