Medical Social Worker

Provide social work services to patients on an intermittent basis in their place of residence Assess patient/family system needs Develop and implement treatment plans in accordance with departmental and medical center policies Perform bio-psychological assessment interviews with patients, families, or significant others Provide crisis intervention and treatment to adults, children, and families in emergency situations Provide assessment, counseling, information/referral, and consultation to physicians and the healthcare team Participate in development of the Plan of Care and prepare clinical and progress notes Submit assessments and notes timely Interpret patients' conditions professionally and recommend appropriate psychosocial intervention and/or treatment plans Provide services under physician orders and the plan of care, under supervision of the Director of Clinical Services/Nursing Supervisor Requirements A Master's or Doctoral Degree from a school of social work accredited by the Council on Social Work Education Two (2) years of Social Work experience in a Home Health setting Passion to serve and help others live their best lives possible Active CPR certification 1-2 years of experience supervising clinicians in a Home Health Agency environment Experience using Electronic Medical Records (EMR) Core Competencies Demonstrates expertise in providing social work services, including assessment, treatment planning, and crisis intervention, while effectively collaborating with healthcare teams and adhering to medical protocols. Holds a Master's or Doctoral Degree in Social Work and possesses experience in home health settings.
