Registered Nurse (RN) for Home Health
This part-time position provides patient nursing services to patients in their home as a Home Health Registered Nurse.
Essential Job Functions:
- Develops and carries out the plan of care in coordination with the patient, family members, physician and home health care team members. Ensures plan of care is appropriate on a continuous basis.
- Coordinates the delivery of services of all members of the home care team as designated by the plan of care and acts as the Case Manager.
- Organizes case conferences with other disciplines as appropriate on at least a weekly basis.
- Provides and documents a thorough assessment during each home visit, including vital signs.
- Works within the parameters of the established Plan of Care (POC) authorized by the provider and modifies only after communication with the provider.
- Provides supervision for LPNs and CNAs. Develops the POC and delegates specific duties, frequencies and parameters for notifying the RN Case Manager.
- Communicates changes in condition and all pertinent information to the provider.
- Completes visits on the day of service and synchs them to the back office daily.
- Updates orders as soon as changes are received or recognized.
- Maintains adequate patient supplies for the patient in their home per patient specific needs.
- Maintains and cares for nursing bag supplies, responsible for all agency supplied equipment.
- Attends 80% of scheduled staff meetings.
- Ensures that the required 12 hours of annual CEUs are completed and proper documentation is submitted to the office.
- Participates in QA/QI activities as requested.
- Completes monthly continuing education topics as assigned.
- Respects patient rights and is respectful of patient and family needs at all times.
- Maintains confidentiality at all times.
- Maintains current licenses and certifications.
- Performs other duties as required.
Required Qualifications:
- Current Colorado RN License or compact state licensure.
- Valid Colorado Driver's License.
- Current BLS.
- National Incident Management System (NIMS) certification; can be obtained at job. Must be completed within 60 days of hire.
Knowledge, Skills and Abilities:
- Ability to effectively communicate verbally and in writing.
- Ability to create trusting, therapeutic relationships while maintaining professional boundaries.
- Is knowledgeable of patient rights.
- Working knowledge of MS Office and other software, able to apply knowledge of computers within secure databases, able to learn new technologies.
- Bilingual, English/Spanish preferred.
- Must pass a pre-employment drug screen and background check.
Education and Experience:
- Successful completion of an accredited Registered Nursing program.
- One year of experience in Home Health preferred.
Physical Requirements, Work Conditions and Environment:
- Moderate physical effort required in the performance of personal care duties, including lifting and/or moving patient with or without the use of an assistive device. The employee is frequently required to use hands to fingers to handle or feel objects, tools or controls; and to reach with hands and arms. The employee is required to walk, stand, sit for long periods of time, talk and hear. Hearing and vision correctable to normal ranges; close-up vision and the ability to adjust focus necessary. Position requires ability to stoop, bend, kneel, crouch, carry, twist and push and/or pull light to moderate amounts of weight. Employee must be able to reach and manipulate objects, tools or controls and to drive. The employee is required to communicate both orally and in writing. Position requires the ability to operate a computer keyboard and standard office equipment at efficient speed. The noise level may be very quiet to noisy depending on the situation.
