Housing Stability Case Manager

Interfaith Works

Interfaith Works is seeking a Case Manager to provide housing-focused case management for individuals experiencing homelessness. You will conduct assessments, develop service and housing plans, coordinate with community resources, and maintain HMIS documentation. Responsibilities include caseload management of ~25 clients, weekly meetings, crisis intervention, and coordination with Continuum of Care. Strong boundaries, judgment, and trauma-informed approach are essential for success. #J-18808-Ljbffr
