This position operates remotely, allowing for flexibility while maintaining a strong connection with the hospice center where you will primarily serve clients.   • Conduct thorough assessments of clients' social, emotional, and environmental challenges to develop tailored service plans that address their unique circumstances and needs.

  • Engage in individual and family counseling sessions, focusing on grief and loss, emotional distress, and coping mechanisms for both patients and their families.

  • Implement crisis intervention strategies and develop safety plans for clients experiencing acute stress or emotional crises.

  • Work collaboratively with clients to identify and connect them with community resources that can provide additional support, including financial assistance, housing, and respite care.

  • Facilitate discussions and meetings with family members to help them understand hospice services and guide them in navigating their loved one’s care.

  • Participate actively in case conferences and team meetings to coordinate care and share insights on patient progress and emerging needs.

  • Engage in community outreach to promote awareness of hospice services and resources available to families facing life-limiting conditions.

The primary clientele for this role includes individuals receiving hospice care, often facing terminal illnesses, and their families who may be experiencing significant emotional turmoil, grief, and challenging transitions. You will address complex issues ranging from end-of-life decisions to bereavement support, focusing not only on the patient but also on providing guidance and care for their families.

  • A Master’s degree in Social Work from an accredited program is required.

  • Active and valid licensure as a Licensed Clinical Social Worker (LCSW) in Texas.

  • Strong foundational knowledge of hospice care principles, end-of-life issues, and bereavement processes.

  • Demonstrated skills in crisis intervention and effective communication.

Compensation and Benefits\n
