- Perform assessments for patients in both inpatient and outpatient scenarios, formulating personalized treatment strategies
- Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality
- Work collaboratively with nurses, physicians, case managers, and fellow rehabilitation professionals to optimize patient outcomes
- Educate patients and their families on therapeutic objectives, plans for discharge, and exercises for home reinforcement
- Accurately document evaluations, treatment provided, and progress achieved in compliance with institutional and legal standards
- Engage in care team rounds and interdisciplinary meetings to ensure a cohesive approach to patient care
- Doctor of Physical Therapy (DPT) or equivalent qualification from an accredited institution
- Current state licensure as a Physical Therapist
- CPR certification highly preferred
- Specialized certifications (e.g., in geriatrics or neurology) are advantageous but not mandatory
- Prior relevant experience is strongly preferred
- Competence in treating patients with intricate medical and mobility challenges
- Excellent communication and teamwork abilities
- Proficiency in utilizing digital documentation and clinical charting systems
